This web-based app is for high-school aged students with disabilities with an IEP. An IEP (Individualized Education Program) details your special education services. After completing a brief 10 minute survey, the site produces an individualized 'Draft Transition Plan', based on your answers. This site is intended to serve as a guide to North Dakota students, parents and educators on transition planning based upon requirements of the Individuals with Disabilities Education Act and North Dakota state law.
